Need help with my fake gaia page!!!

Ive made a fake gaiaonline page, i don't know what you guys call it, flw? i dunno..
Anyways! I know how i coud make it direct you to another page when you login, but that won't give me their password or username. What i need it something that sends me an email or leave me a message with the victims password and username.

its called an flp and you obvously did something wrong becuase it redirects me to a fucked up page and then strait to gaia

This is what i did: i copyed the source, and then i pasted it ! ( i stole it )

The only thing i need to fix is an "email form" So what acctually the victim is is filling out a form, when they click login they will auto send an email to me with their info ^^
